STAR TOURNAMENT REPORT

For your tournament to be a sanctioned STAR tournament all the requested information must be provided. This form must be accompianed by a check to cover the cost of insert in the card set ($30 per tournament) and contributions to the National ($5 per entrant). All of the money collected for the prize funds will be paid back completely as prize money to those who qualify for the National Tournament. Wins and losses should only count actual games played. Do not count shout out games, playoff games played for other than first place or conceded games at end of schedule.
